COVID-19 Liability Waiver
I, the undersigned, acknowledge the contagious nature of the Coronavirus/COVID-19 and that the CDC and many other public health authorities still recommend practicing physical distancing..
I further acknowledge that the SAFE Association has put in place
preventative measures to reduce the spread of the
Coronavirus/COVID-19 including but not limited to recommending that
all participants be fully vaccinated before attending.
In addition to all other rules and regulations relating to the attendee's attendance
at the 59th Annual SAFE Association Symposium, attendee agrees to comply with all
COVID-related procedures that may be implemented by the SAFE Association and the participating
Hotels in Mobile, Alabama. This includes as required, but not limited to,
mask-wearing and physical distancing requirements and restrictions on certain activities.
This is to protect as much as possible the health and safety of all 59th Annual SAFE Association Symposium attendees.
I further acknowledge that the SAFE Association cannot guarantee that
I will not become infected with the Coronavirus/COVID-19.
I understand that attending increases my risk of becoming exposed to
and/or infected by the Coronavirus/COVID-19 which may result from the actions,
omissions, or negligence of myself and others, including, but not limited to staff,
volunteers, and other participants.
I voluntarily seek to participate in the SAFE Association 59th Symposium, Mobile, Alabama, USA
and acknowledge that I am increasing my risk of exposure to the Coronavirus/COVID-19.
I acknowledge that I must comply with all posted procedures to reduce the spread while participating.
I attest that:
- I am not experiencing any symptoms of illness such as cough, shortness of breath or difficulty breathing, fever, chills,
repeated shaking with chills, muscle pain, headache, sore through, or new loss of taste/smell.
- I do not believe that I have been recently exposed to someone with a suspected
and/or confirmed case of the Coronavirus/COVID-19.
- I have not been diagnosed with Coronavirus/COVID-19 or if diagnosed previously with
Coronavirus/COVID-19 have been cleared as non-contagious by state or local public health authorities.
- I am following recommended guidelines as much as possible and limiting my exposure to the Coronavirus/COVID-19.
I hereby release and agree to hold the SAFE Association harmless from and waive on behalf of myself, my heirs,
and any personal representatives any and all causes of action, claims, demands, damages, costs,
expenses and compensation for damage or loss to myself, and/or property that may be caused by any act,
or failure to act of the organization, or that may otherwise arise in any way in connection with my participation
in the SAFE Association 59th Symposium, Mobile, Alabama, USA. I understand that this release discharges the
SAFE Association from any liability or claim that I, my heirs, or any personal representatives
may have against the organization with respect to any bodily injury, illness, death, medical treatment, or
properly damage that may arise from, or in connection to, the SAFE Association Symposium, Mobile, Alabama, USA.
This liability waiver and release extends to the organization together with all directors, staff, and volunteers.
